Referral Criteria: Patient must be diagnosed with a long term condition to access the service.
Signposting to Other Agencies: Works closely with third sector agencies to provide social support and works closely with health care providers, e.g. district nurses, to assist with any medical conditions. Team works closely with Brecon Cancer Centre.
